SS-31

SS-31 (Elamipretide / Bendavia, D-Arg-2',6'-dimethyltyrosine-Lys-Phe-NH2) is a cell-permeable, mitochondria-targeted tetrapeptide that selectively concentrates at the inner mitochondrial membrane (IMM) due to its alternating aromatic-cationic motif and cardiolipin binding affinity. Cardiolipin is a unique phospholipid found almost exclusively in the IMM where it plays essential roles in maintaining cristae architecture, anchoring electron transport chain (ETC) complexes, and facilitating the formation of respiratory supercomplexes. SS-31 stabilizes cardiolipin interactions with cytochrome c, preventing its peroxidase activity and subsequent release into the cytoplasm (a key apoptotic trigger). Research has demonstrated SS-31's ability to optimize electron transport chain efficiency (particularly Complex III and IV), reduce mitochondrial ROS production by up to 60%, prevent ischemia-reperfusion injury in cardiac, renal, and cerebral models, reverse age-related mitochondrial dysfunction, and restore mitochondrial membrane potential. Clinical trials have evaluated its efficacy in heart failure, Barth syndrome, age-related macular degeneration, and primary mitochondrial myopathy. Research also explores its potential to reverse skeletal muscle aging and improve renal function in aging kidneys.
