Tirzepatide

Tirzepatide is a first-in-class dual GIP and GLP-1 receptor agonist peptide featuring a novel unimolecular design based on the 39-amino acid GIP sequence with engineered GLP-1R cross-reactivity. A C20 fatty diacid moiety linked via a linker at Lys20 enables albumin binding and once-weekly dosing pharmacokinetics. The dual agonism produces synergistic metabolic effects: GIP receptor activation enhances pancreatic beta-cell function and insulin sensitivity in adipose tissue, while GLP-1R activation mediates appetite suppression, gastric emptying delay, and glucose-dependent insulin secretion. The SURPASS and SURMOUNT clinical trial programs demonstrated superior glycemic control compared to all other tested agents, along with unprecedented body weight reductions of up to 22.5% in some cohorts. Research applications include studies on dual incretin signaling synergy, beta-cell preservation, adipose tissue insulin sensitivity, hepatic de novo lipogenesis inhibition, and cardiovascular risk biomarker modulation.
